To become a patient of Artemis in the City and Dr. Heard, you must complete a new patient intake assessment.
The New Patient Intake includes a thorough health history review, dietary recall, questionnaires, review of your health goals and what would like to achieve through working with a clinical nutritionist as well as review of a recent comprehensive bioscreen diagnostic lab test.
Comprehensive bioscreen diagnostic lab test results are required in order to complete the nutrition intake assessment. Lab tests can be ordered through your medical doctor or Artemis in the City and are not included in the listed cost.
The New Patient Intake Option 3 includes the following:
- 1 one-on-one 2-hour (120 min) nutrition intake assessment scheduled with Dr. Danielle Heard, DCN, MS, MS, CNS, LDN, CDN, CKNS, INHC.
- 12 one-on-one 1-hour (60 min) nutrition consultation follow up appointments with Dr. Danielle Heard, DCN, MS, MS, CNS, LDN, CDN, CKNS, INHC. This is a 6 month nutrition coaching program. Sessions are ideally scheduled every other week. It includes ongoing email support between sessions, health articles and handouts and healthy delicious recipes.
- **48 hour turn-around response time
A New Patient Intake is a requirement for all new patients in order to review past and current health information. This New Patient Intake Option 3, provides 13 sessions to cover an intake, plan of recommendations and eleven follow up appointments. If you have ongoing chronic health conditions or planning pregnancy and plan also nutrition therapy with additional diagnostic lab testing, it is recommended that you consider starting with the New Patient Intake Option 2, Option 3 or Option 4 that includes ongoing nutrition coaching which will provide for ongoing monitoring, evaluation and follow up.
If you are self-paying for services, a New Patient Intake must be paid for in advance in order to reserve the session time in Dr. Heard's calendar. If you have health insurance, please check the list of plans in which Dr. Heard is a provider. Benefits will need to be checked.
If for any reason you cancel an appointment without 48-Hour notification, the payment is non-refundable. If you have a personal or family emergency we will work with you to make up the session. Life happens to all of us!
Dr. Heard uses an integrative and functional nutrition approach that is based on both scientific research and Eastern and Western dietary theories to help patients achieve optimal health and wellness. This integrative approach is based on bio-individuality and body intuition as no one diet is right for everyone due to differences in genetics, ethnicity, metabolism and lifestyle.
Sessions are conducted via tele-health web conferencing through a HIPAA compliant Electronic Health Records (EHR) portal.
